Simply Elegant Media Day - Model Release Form

I. Parties Involved

This Model Release Agreement (the "Agreement") is made and entered into on the date stated above, by and between:

Photographers & Content Creators: Tim and Melonie McEver (Hereinafter referred to as "Photographer") and MODEL.

II. Release and Consent

For good and valuable consideration, receipt of which is hereby acknowledged, the undersigned Model, or legal guardian if the Model is a minor, hereby irrevocably grants to the Photographer, their licensees, successors, and assigns, the perpetual right to photograph and/or videotape the Model and to use, reproduce, publish, exhibit, and distribute any and all such photographs, video recordings, and other content created (the "Content") in which the Model may be included, in whole or in part, for any lawful purpose whatsoever.
